xG Health Solutions launches xGCare, a revolutionary care management platform

xG Health Solutions, a leading healthcare management and redesign company, is pleased to announce the launch of xGCare, a care management platform that helps healthcare organizations improve quality of care, enhance the patient experience and reduce overall costs.

xGCare combines the in-depth experience of xG Health’s expert clinicians with population health-management process improvements developed and refined over 20 years by Geisinger Health System.
